Is CCM covered by insurance? 

CCM is a Medicare-covered service for patients with two or more chronic conditions expected to last at least 12 months. 

There is no copay for most patients with supplemental coverage or secondary insurance. 

If you have standard Medicare Part B only, a small coinsurance may apply (usually under $10 per month).

Most Medicare Advantage plans also cover CCM.

How do I enroll in CCM?

Participation is completely voluntary—you can opt in or out at any time.

Does CCM replace my regular office visits? 

No. CCM is designed to support your regular care, not replace it. You’ll still have your normal in-office or virtual visits as needed, but CCM helps manage your care between appointments. 

What kinds of conditions qualify for CCM? 

Common examples include diabetes, high blood pressure, heart disease, COPD, asthma, arthritis, kidney disease, depression, and other long-term conditions that require ongoing management. 

How often will I hear from my care team? 

You’ll receive at least one check-in per month by phone or secure message, and more frequently if needed. 

How much will CCM cost me? 

For most patients, CCM is fully covered by Medicare and supplemental insurance. If you have Medicare Part B only, your standard 20 % coinsurance may apply (typically less than $10 per month). 

Can I stop CCM if I change my mind? 

Yes. CCM is completely voluntary. You can opt out at any time by notifying the clinic.
