Geriatric care at Millenium Elite Primary Care consists of high-quality, patient-centered care for the elderly population. The practice also offer a program called Chronic Care Management (CCM) which is a Medicare-supported program designed for patients who live with one or more chronic conditions such as diabetes, hypertension, heart disease, asthma, or arthritis. It provides ongoing support between office visits so your care team can help you manage your health, medications, and lifestyle goals more effectively. Some of the benefits are: monthly check ins via phone or more if needed, continuous monitoring and early detection of changes in your health, personalized coaching and education on your conditions, improved coordination between all your healthcare providers, fewer ER visits and hospitalizations.

What happens during geriatric care appointments?

We complete a comprehensive physical exam during geriatric care appointments at Millenium Elite Primary Care. We check your vital signs, weigh you, and review your symptoms, lifestyle, medical history, and medications.

We assess your overall health and may examine your eyes, ears, throat, abdomen, neck, limbs, breathing, and heartbeat. We offer vision and hearing screenings, mental health/cognition evaluations, labs testing, heart function testing, diagnostic tests and referral for lung function tests, bone scans, and more.

If you need treatment, we tailor a plan that maximizes your physical and mental health outcomes. We address any concerns or questions you have and may recommend preventive medicine options like vaccinations, dietary supplements, or healthy lifestyle adjustments. 

How often should I schedule geriatric care appointments?

Visit with us anytime you have questions about your well-being, need help managing a chronic disease, or become ill or injured. See us yearly for your annual physical and more often (twice yearly or every few months) if we recommend it based on your medical history.
